The World Of Motherhood

Is surgical treatment of obesity a viable option?

Physical beauty has been excessively glorified by the society. Though obesity is by no means a preferred health condition to be in, the fact that society discriminates people who are not slim leads to psychological conditions like low self esteem, depression and eating disorders.

There is no shortcut to reduce weight. Reducing weight with the aid of weight loss medications often leads to side effects, which can cause their own set of complications. Alternate remedies for treating obesity require a protracted effort at controlling one’s desire to eat. There are herbal weight loss solutions that involve natural weight loss and these can be fairly effective in reducing weight. But following those programs also requires a concerted effort at restraint and control.

Recently, surgical treatments of obesity have been touted as a quick fix solution for obesity. Generally, surgeons use different procedures to treat obese individuals:

* Gastric Restrictive Surgery is a process by which the size of the stomach is reduced.
* Vertical Banded Gastroplasty involves stapling of the stomach to reduce its capacity roughly to 15ml or ½ ounce. This leads to dividing the stomach in two pouches - one small that is stapled and the other unstapled and bigger. The food from the esophagus enters the small pouch and the bigger pouch receives small amounts of food from it. Over eating produces nausea, vomiting or pain and therefore restricts over eating.
* Laparoscopic Gastric Binding attempts to achieve the same goal as the above but involves placing a band around the upper portion of the stomach. This procedure is becoming more popular than the earlier surgery due to the involvement of latest technology. It uses a laparoscope, a slender endoscope inserted through a small incision in the abdominal wall in order to perform surgery.
* Gastrointestinal Bypass Surgery is a variation of restrictive surgery where a portion of the stomach in stapled off. A portion of the small intestines is attached directly to the stapled portion bypassing the small intestines.
* Biliopancreatic Bypass is a process wherein the juices of liver and the pancreas that help in digestion are diverted to the far end of the small intestine, near the large intestines. Food enters the stomach and rapidly crosses the small intestines where absorption of nutrients occurs. The remaining bulk passes over to the large intestines where excess water is removed from the stool. This leads to a significant mal-absorption leading to nearly 80% weight loss.

Physicians recommend surgery only in cases where the body weight is at least 45kgs or 100 lbs above the ideal body weight. Although there has been a marked development in the surgical procedures, surgery as weight loss solutions is beset with significant problems. All surgeries for obesity treatment reduce the absorption of nutrients, which causes mineral and vitamin deficiencies, ultimately leading to anemia. Mal-absorption of nutrients often leads to the weakening of bones. Some conditions caused by such surgeries may involve a life long treatment. Inferences of studies reveal that nothing can be said with certainty about the efficacy of surgical treatment for obesity. It may result in a significant weight loss but the evidence for it is extremely limited.
